The form below is designed so that teachers can prepare and submit a brief year-end evaluation for their students. The form can also be used for a quarterly evaluation. These forms are completed by teachers, and copies are mailed to parents along with the final Report Card, which are mailed to the parents in middle to late May of each year.

    ​Evaluation Categories


    Assignments: This student completes homework and classwork in a timely manner. This student keeps up with reading assignments. This student is careful and diligent in the completion of homework assignments on a consistent basis.

    Organization: This student manages his or her assignments well. This student does not lose or forget assignments. This student is conscientious about contacting the teacher regarding late, make-up, and/or missed assignments.

    Conduct: This student is courteous, cheerful, and respectful of the instructor and other TLC students. This student is careful to abide by the TLC dress code and the rules for study hall. This student is consistently punctual.

    Academics: This student applies himself or herself both at home and during class. This student has good study habits, as evidenced by general homework and classwork quality and overall preparedness, as well as by test and quiz performance.

    Character: This student exemplifies mature and Christlike behavior. He or she is kind, patient, diligent, and loving. He or she is a good influence on others.


    Note to Parents

    This Evaluation does not replace the TLC Report Card, which is a far more precise measure of your son or daughter’s academic performance, but this form was completed by this particular instructor to provide you with informal, supplemental, and more specific information regarding your son or daughter’s particular strengths and weaknesses.

    In some cases, recommendations are offered by the instructor(s) on potential areas for improvement either over the summer or in the future. If you would like to discuss your student and/or anything in this informal evaluation in greater detail, please feel free to contact the particular TLC instructor directly.
